How to add or update your Booster Shot info.

Step 1: Click update Booster Vaccination Dates.

Step 2: Complete the requested info: Provider, Date and Upload an image of your booster card (if desired). If you do not have a picture of the booster shot card you can add it later. 

Step 3: You can either take a photo of your booster card, add it from your photo gallery on your phone or upload an image or pdf file.

Step 4: Click submit if everything is correct.

Step 5: Once you have updated the photo and the booster shot date they will appear immediately.

Step 6 (if needed): To add additional booster shots click the update Booster Vaccination Date button. Next, click the plus sign (+) and then yellow submit button after the details are added.
